? Responsibilities
• Review medications during patient transfers and assess medication supply needs
• Contact patients before transfer regarding medication requirements
• Provide medication counselling in-person and virtually
• Perform medication reconciliation
• Review discharge prescriptions and verify new medication orders
• Identify inappropriate medications or dosages where applicable
• Support medication supply duties and follow up on medication preparation status
• Perform other duties as assigned based on operational requirements

? Requirements
• Min. degree in Pharmacy
• Fully registered with the Singapore Pharmacy Council
• Good communication, interpersonal and organisational skills
• Able to work independently and collaboratively within a multidisciplinary team
• Able to commit to weekend shifts
