- Direct work with patients and families, providing necessary assistance
- Support in case management through providing patient escort and home visit suppor
- Participate in work process improvements and quality improvement projects
- Facilitate patient care and experience
- Provide support on the administrative and operational running of the department
- Supporting in programmes and projects
Requirements:
- Min diploma in social work or a related field
- Prior relevant experience will be advantageous
